What should you do if you are injured on the job?
- Report all injuries to your supervisor
- Seek medical attention as outlined below
- Complete an Incident Report for your supervisor
In an emergency, go to the nearest emergency facility. Access to emergency facilities is not restricted.
In a non-emergency situation, seek and obtain care as listed below.
In Alabama, District of Columbia, Illinois, Indiana, Iowa, Kansas, Kentucky, Maryland, Missouri, North Carolina, South Carolina, Tennessee, Virginia and West Virginia**:

Physician Panel States
◈ In Pennsylvania, Tennessee and Virginia:
Physician Panels will be created utilizing the Coventry and Procura PPO Networks outlined above. The physician panel should be provided by your employer at the time of injury.
Pharmacy Network Search
Injured employees have the ability to receive a first-fill prescription for up to 14 days from the date of injury without a pharmacy card. Complete the
First Fill Form and take the form to a network pharmacy of your choice to obtain medication(s) related to your injury. Upon acceptance of the claim, a pharmacy identification card will be mailed to you within 24 hours.
